| At McMahon-Coyne-Vitantonio Funeral Homes our prices
are second to none. In fact, we challenge you to find a better
price. The goods and services shown below are those
we can provide to our families. You may choose the items you
desire, however any funeral arrangements you select will include a
charge for our services. If legal or other requirements
dictate that you must buy any items you do not specifically ask for,
we will explain the reason in writing on the statement we provide
describing the funeral goods and services that are required.
*These prices are effective April 1, 2005 and are
subject to change without notice.

comply with the Federal Trade Commission's Trade Regulation Rule - Funeral Industry Practice 16 CFR part 453. |

| SERVICES OF FUNERAL DIRECTOR & STAFF |

| Includes: General and
administrative expenses; arrangements conference; twenty-four hour
accessibility; planning; supervising and coordinating service
details with those involved in the funeral service; securing,
preparing and filing necessary permits, certificates, authorizations
and consents; clerical and record keeping. This fee for our
basic service and overhead will be added to the total cost of the
funeral arrangements you've selected. |

| EMBALMING |
$425.00 |
| Embalming is not required by law except
in certain cases. However, embalming may be necessary if you
select certain funeral arrangements, such as a funeral with viewing.
If you do not want embalming, you usually have the right to choose
an arrangement that does not require you to pay for it, such as
direct cremation or immediate burial. |

| FORWARDING OF DECEASED TO ANOTHER FUNERAL HOME |

| This charge includes: General and
administrative expenses, arrangements conference; twenty-four hour
accessibility; planning, supervising and coordinating service
details; securing, preparing and filing necessary documents;
clerical and record keeping; embalming; other preparation of the
deceased; transfer of deceased; use of hearse and local
transportation of the decease3d to the airport. (This does not
include shipping charges.) |

| DIRECT CREMATION |

| This charge includes: General and
administrative expenses, arrangements conference, twenty-four hour
accessibility; securing, preparing and filing necessary documents;
clerical and record keeping; care of deceased, transfer of deceased
to funeral home and to crematory; use of removal vehicle; crematory
charges; a cremation container which is a crematory requirement.
If you want to arrange a direct cremation, you can use an
alternative container. Alternative containers encase the
deceased and can be made of materials such as fiberboard or
composition materials (with or without outside covering). The
containers we provide are Fiberboard (cardboard). |
